What you will do

• Lead group support sessions and lifestyle classes following clinical protocols to aid cardiac rehab patients.
• Create emotionally safe virtual environments utilizing active listening and conflict de-escalation skills.
• Collaborate with a transdisciplinary team across nutrition, stress management, and exercise elements.
• Empower patients to take ownership of their rehabilitation and maintain accountability.
• Innovate and improve the digital care delivery model in direct collaboration with the founders.
• Guide patients through the digital health program, including troubleshooting wearables and tablets.

Requirements

• Master’s Degree in social work, counseling, or a related mental health field.
• Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), or other mental health professional license.
• Minimum 2+ years of relevant clinical or facilitation experience in behavioral health or chronic disease settings.
• Must be based in the United States.
• Strong proficiency with mobile technology and ability to troubleshoot patient hardware issues.

Nice to have

• Prior experience delivering Dean Ornish's ICR / OLM program.
• Spanish language proficiency.
• Certification as a Registered Yoga Teacher (RYT) or Registered Dietician.
• Experience with clinical trials, healthcare research, or telehealth.
