Clinical Care Navigator (Mental Health)

Location: Fully remote, but candidates must be physically located within the United States. Required shifts are Tuesday–Saturday or Sunday–Thursday, 9:30am–6pm Pacific Time, with equivalent hours across US time zones. Weekend shifts include a 10% pay differential.

Salary: $41 per hour base rate for the full-time position.

What you will do

• Provide timely, evidence-informed clinical support for emotional, behavioral, situational, and crisis-related concerns.
• Conduct risk assessments and safety planning, using de-escalation, rapport building, and evidence-based coping interventions.
• Connect clients with Lyra care options, providers, clinical resources, and assisted booking services.
• Conduct follow-up outreach to support continuity of care, reassess risk, and address barriers to treatment.
• Respond to a high-volume, high-acuity inbound call queue while documenting interactions and following clinical protocols.
• Collaborate with relevant Lyra teams to support safe and effective client care.

Requirements

• Master’s degree in clinical social work or equivalent, with independent clinical licensure such as LCSW, LMFT, LPC, or LPCC, or an RN license.
• RNs need two years of behavioral health experience, including risk assessment and safety planning.
• Other licensed clinicians need two years of post-license experience; EAP or clinical call center experience is preferred.
• Significant experience managing high-acuity clinical situations, including suicidality, homicidality, self-injury, and mandated reporting.
• Bilingual Spanish-speaking ability and full-time residence in the United States are required.
