What you will do

• Conduct comprehensive telehealth video visits and occasional home-based assessments for members.
• Perform diagnostic tests, medication reviews, and assessments for quality of care measures like STARS/HEDIS.
• Collaborate with a multidisciplinary team including physicians, social workers, and clinical nurses to coordinate care.
• Utilize and provide feedback on the internal electronic health information system to improve clinical workflows.
• Support member care coordination by interfacing with PCPs, specialists, and family caregivers.
• Participate in infrequent weekend on-call rotations to support clinical triage as required.

Requirements

• Active and clear RN and APRN license in Georgia and APRN certification required at time of hire.
• Active BLS certification required at time of hire.
• 3+ years of experience working in outpatient clinical practice.
• Experience in primary care, family medicine, geriatrics, or palliative care.
• Experience performing Medicare annual wellness visits or in-home comprehensive visits.
• Proficiency in performing visits over telehealth video platforms.

Nice to have

• Prior experience in home-based clinical practice.
• Understanding of managed care, including STARS/HEDIS measures and clinical coding.
• Bilingual proficiency in Spanish or Haitian Creole.
• Multi-state licensure or willingness to obtain additional licenses.
