What you will do

• Build trusting relationships with patients through telephonic and virtual interactions.
• Collaborate with an interdisciplinary team of clinicians and coordinators to ensure cohesive, patient-centered care.
• Conduct screenings and develop personalized action plans to address socioeconomic and psychosocial barriers.
• Provide emotional support to caregivers and assist in navigating complex family support issues.
• Facilitate end-of-life planning discussions and coordinate transitions to palliative or hospice care.
• Monitor long-term care goals and arrange necessary referrals for complex patients.

Requirements

• Master’s degree in social work and active state license required.
• Minimum of 3 years of experience in medical social work, case management, or care coordination.
• Proven success in building relationships with patients and interdisciplinary teams.
• Ability to multitask in a fast-paced, technology-driven environment.
• Must be based in the USA.
