Social Work Case Manager

Salary: $70,000–$85,000 annually

What you will do

• Build trusting relationships with patients and caregivers through telephonic and virtual engagement.
• Conduct screenings, identify socioeconomic and psychosocial barriers, and create personalized action plans.
• Coordinate patient care with clinicians, case managers, community health workers, coordinators, and behavioral health specialists.
• Support caregivers experiencing burnout and help patients and families navigate complex support needs.
• Facilitate long-term care, advanced care, palliative care, and hospice transition discussions.
• Coordinate referrals, local resources, and services while monitoring outcomes and balancing competing priorities.

Requirements

• At least 3 years of experience in medical social work, case management, or care coordination.
• Master’s degree in social work and an active state license.
• Experience building relationships with patients and interdisciplinary teams.
• Ability to manage multiple competing priorities, document information while communicating, and work in a changing environment.
• Comfort working remotely and supporting patients virtually.

Nice to have

• Bilingual Spanish proficiency.
• Health insurance experience, especially Medicare Advantage.
• Experience working with older adults.
