What you will do

• Implement ACO initiatives and wellness programs in partner practices.
• Train office staff and physicians on value-based care processes and technologies.
• Develop and nurture relationships with healthcare providers and system executives.
• Assess practice training needs and provide ongoing support.
• Relay practice concerns and improvement opportunities to leadership.
• Ensure practices meet performance metrics and transformation goals.

Requirements

• Must be a resident of New Jersey.
• Bachelor’s degree or 4-6 years of relevant medical practice management experience.
• 3 years of experience with Electronic Health Records (EHR).
• Ability to travel up to 25% within the New Jersey market.
• Strong communication, organizational, and project management skills.
• Proficiency in process mapping and workflow analysis tools.
