What you will do

• Conduct in-person outreach to identify and engage eligible members.
• Schedule intake appointments and assist members with required documentation.
• Represent the organization at community coalition meetings and collaborative events.
• Build and maintain relationships with local community partners and stakeholders.
• Document all outreach attempts and outcomes in Athena and internal trackers.
• Perform additional tasks as needed to support the scaling of the organization.

Requirements

• Must reside in Inyo, Mono, or Alpine County, CA.
• 1-3 years of experience in healthcare or community outreach.
• Bilingual proficiency in English and Spanish is required.
• Proficiency with electronic health records, specifically Athena.
• Access to reliable transportation for in-person outreach.
• Ability to work a flexible schedule, including evenings and weekends.
