Family Resource Specialist

TL;DR

Family Resource Specialist: Providing direct non-clinical support to patients and families by connecting social needs to community and care-team resources with an accent on social services navigation, referrals, and closing non-medical drivers of health gaps. Focus on building and maintaining community partnerships, documenting partnership agreements, and supporting families in accessing programs for children with complex medical and behavioral health needs.

Location: Remote (Missouri). Work schedule 8:00am–5:00pm CST, Monday–Friday with 2 days determined with supervisor being 10:00am–7:00pm. Quarterly travel to Houston, TX.

What you will do

• Identify and connect families/caregivers to community resources and support services.
• Provide information, referrals, and non-clinical assistance to link families to medical, psychosocial, and basic needs services.
• Advocate with community agencies and internal staff to reduce service gaps and remove barriers to care.
• Support families in navigating systems by providing education, guidance, and tools to access needed programs.
• Build and maintain an internal community resource library (non-clinical materials) and document memoranda of understanding (MOUs) for partnerships.
• Participate in scheduled interdisciplinary consultations and coordinate linkage to community-based and school-based services.

Requirements

• Must be living in Missouri.
• Bachelor’s degree or higher in a healthcare, behavioral health, or social services related field (preferred).
• Certification as a Community Health Worker (highly desired).
• Experience navigating social service agencies and providing resourcing/telephonic services.
• Bilingual Spanish required.
• Reliable transportation, valid Driver’s License, and ability to provide a copy of current auto insurance certificate.
