Compensation: $130,000–$170,000 total compensation

What you will do
- Conduct virtual assessments for patients with hypertension and develop individualized treatment plans.
- Manage statin therapy to mitigate cardiovascular disease risk for patients with diabetes.
- Collaborate with an interdisciplinary team of physicians, pharmacists, and social workers to coordinate patient-centered care.
- Provide clinical escalation support and advisor services for specialty clinic staff.
- Utilize and provide feedback on a proprietary electronic health information system to improve clinical interfaces.
- Maintain accurate patient medical records in compliance with legal and ethical guidelines.
Requirements
- Active RN and APRN or Physician Assistant license.
- Willingness to obtain and maintain multiple state licenses (company operates in 29 states).
- 5+ years in outpatient clinical practice.
- 2+ years concentrated in primary care or a relevant subspecialty (e.g., cardiology, nephrology, endocrinology).
- Availability to work Monday-Friday, 8:30 am–5:00 pm MST or PST (EST and CST also considered).
Nice to have
- Familiarity with AI-assisted clinical workflows.
- Proficiency in telehealth technology and electronic health records (EHR).
- Experience in virtual care and an understanding of managed care (STARS/HEDIS measures).
- Proficiency in Spanish.
