What you will do

• Conduct virtual adult and pediatric wellness visits and health assessments directly to patients in their homes.
• Perform comprehensive health risk assessments, medication reviews, and cognitive screenings.
• Collaborate with in-home clinical team members to evaluate vital signs and coordinate blood draws.
• Provide care coordination, including preventive interventions, specialist referrals, and community resource management.
• Document clinical findings and communicate via HIPAA-secure messaging apps.

Requirements

• Active Family Nurse Practitioner License and Board Certification in North Carolina
• Prior experience performing wellness visits.
• Technologically savvy with proficiency in laptops, mobile devices, and secure charting tools.
• Strong written and verbal communication skills.
• Ability to pass a national background check and clinical license search.
