What you will do

• Provide virtual primary care visits via telehealth, including comprehensive assessments and acute care interventions.
• Diagnose and manage chronic conditions (e.g., hypertension, diabetes, COPD) and develop individualized care plans.
• Order and interpret diagnostics, manage medication prescriptions, and coordinate follow-up care.
• Deliver health coaching and patient education to empower members in managing their health.
• Collaborate with multidisciplinary teams, including specialists, pharmacists, and care coordinators.
• Utilize and provide feedback on the electronic health platform to improve the provider and patient experience.

Requirements

• Active APP license (NP/PA) in AL, AZ, AR, CO, FL, GA, HI, IL, IN, KY, MS, MO, NC, OH, PA, SC, TN, or TX.
• Must be based in the USA.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of experience in primary care, urgent care, or internal medicine.
• Minimum 6 months of experience working as a Nurse Practitioner in the D2Me program.
• Active BLS certification and willingness to obtain additional licenses and DEA certification within 90 days.
