Advanced Practice Provider (Geriatrics): Delivering comprehensive clinical care to complex geriatric patients via telehealth with an accent on chronic disease management and interdisciplinary collaboration. Location: Remote (USA). Must be based in the US and hold active licenses in AZ, CO, HI, and/or OR. Working hours: 8:30am-5:00pm MST or PST.
Salary: $120,000–$150,000 base range (Total compensation $130,000–$170,000 including bonus).

What you will do
- Deliver exceptional clinical care to complex patients using video telehealth visits.
- Guide and oversee medical management for patients within the Gold Care program.
- Collaborate with PCPs, specialists, pharmacy teams, and patient support systems to coordinate care.
- Perform comprehensive assessments, post-acute transition support, and medication reviews.
- Maintain accurate medical records in compliance with legal and ethical standards.
- Support other service lines and participate in infrequent weekend on-call rotations.
Requirements
- Active and clear RN and APRN or Physician Assistant license in AZ, CO, HI, and/or OR required at time of hire.
- 3-5+ years of experience as an APP in primary care, internal medicine, family medicine, geriatrics, or palliative care.
- Proficiency in telehealth technology and electronic health records (EHR).
- Active BLS certification at time of hire.
- Must be available to work 8:30am-5:00pm MST or PST.
Nice to have
- Virtual care experience.
- Experience working with complex, frail, or vulnerable populations in Skilled Nursing Facility (SNF) settings.
- Proficiency in Spanish.
