Advanced Practice Provider (Geriatrics)

Location: Remote (USA). Must be based in the US and hold active licenses in AZ, CO, HI, and/or OR. Working hours: 8:30am-5:00pm MST or PST.

Salary: $120,000–$150,000 base range (Total compensation $130,000–$170,000 including bonus).

What you will do

• Deliver exceptional clinical care to complex patients using video telehealth visits.
• Guide and oversee medical management for patients within the Gold Care program.
• Collaborate with PCPs, specialists, pharmacy teams, and patient support systems to coordinate care.
• Perform comprehensive assessments, post-acute transition support, and medication reviews.
• Maintain accurate medical records in compliance with legal and ethical standards.
• Support other service lines and participate in infrequent weekend on-call rotations.

Requirements

• Active and clear RN and APRN or Physician Assistant license in AZ, CO, HI, and/or OR required at time of hire.
• 3-5+ years of experience as an APP in primary care, internal medicine, family medicine, geriatrics, or palliative care.
• Proficiency in telehealth technology and electronic health records (EHR).
• Active BLS certification at time of hire.
• Must be available to work 8:30am-5:00pm MST or PST.

Nice to have

• Virtual care experience.
• Experience working with complex, frail, or vulnerable populations in Skilled Nursing Facility (SNF) settings.
• Proficiency in Spanish.
