What you will do

• Conduct telephonic outreach and scheduled video visits for patients post-hospital discharge.
• Perform medication reconciliation and symptom screening.
• Order necessary follow-up labs, medications, or supplies.
• Build trust with patients to explain the care model and connect them to essential services.
• Coordinate care across hospital, pharmacy, and community provider settings.
• Collaborate with the nursing team to meet population health performance targets.

Requirements

• Active Nurse Practitioner license in South Carolina (SC)
• 5+ years of post-graduate clinical experience in Primary Care or Geriatrics
• Direct experience engaging Medicare and Medicaid populations with complex conditions.
• Comfort in a digital-first environment and virtual care delivery.
• Excellent written and verbal communication skills.
• Ability to work 32-36 hours per week, including weekends on a monthly basis.

Nice to have

• Active licenses in TX, NJ, PA, IL, NY, MA, MN, or LA.
• Bilingual in English and Spanish.
• Experience with Google Suite and Slack.
