What you will do
- Conduct virtual consultations and thorough assessments for patients with hypertension.
- Develop individualized treatment plans, including medication management and lifestyle modifications.
- Proactively manage statin therapy to mitigate cardiovascular disease risk.
- Collaborate with an interdisciplinary team of physicians, pharmacists, and social workers to coordinate holistic care.
- Act as a clinical advisor and provide escalation support for specialty clinic staff.
- Utilize and provide feedback on a proprietary electronic health information platform to improve the user interface.
Requirements
- Multi-state licensure is required, with willingness to maintain additional licenses.
- 5+ years of experience in outpatient clinical practice focused on hypertension and ASCVD.
- Minimum 2 years concentrated in primary care or a relevant subspecialty (e.g., cardiology, nephrology).
- Active and clear APP license and certification.
- Active BLS certification.
- Proficiency in telehealth technology and electronic health records (EHR).
Nice to have
- Virtual care experience.
- Proficiency in Spanish.
- Understanding of managed care, including STARS/HEDIS measures and clinical care gaps.
