What you will do

• Conduct virtual consultations and thorough assessments for patients with hypertension.
• Develop individualized treatment plans, including medication management and lifestyle modifications.
• Proactively manage statin therapy to mitigate cardiovascular disease risk.
• Collaborate with an interdisciplinary team of physicians, pharmacists, and social workers to coordinate holistic care.
• Act as a clinical advisor and provide escalation support for specialty clinic staff.
• Utilize and provide feedback on a proprietary electronic health information platform to improve the user interface.

Requirements

• Multi-state licensure is required, with willingness to maintain additional licenses.
• 5+ years of experience in outpatient clinical practice focused on hypertension and ASCVD.
• Minimum 2 years concentrated in primary care or a relevant subspecialty (e.g., cardiology, nephrology).
• Active and clear APP license and certification.
• Active BLS certification.
• Proficiency in telehealth technology and electronic health records (EHR).

Nice to have

• Virtual care experience.
• Proficiency in Spanish.
• Understanding of managed care, including STARS/HEDIS measures and clinical care gaps.
