Location: Must reside in AZ, AR, CA, CO, DC, FL, GA, IL, MA, MI, NV, NH, NJ, NY, NC, OH, PA, TX, or VA.

Salary: $116,400–$152,775 per year.

What you will do

• Perform preventative care screenings and medication reconciliation.
• Diagnose medical conditions and provide health coaching, referrals, and treatment.
• Outreach to chronically ill members based on historical health data.
• Engage members with low probability of traditional PCP visits.
• Interact with members virtually for assessment and treatment.
• Cross-train into urgent care and virtual primary care service lines.

Requirements

• PA or DNP, FNP, ENP from an accredited program.
• Board Certification (AANP, ANCC, or NCCPA).
• Bilingual: Spanish fluency (read, write, & speak) is required.
• 3+ years of experience in outpatient Family Medicine, Internal Medicine, or Primary Care.
• PA: Licensed in Georgia and one of FL, TX, or OH.
• NP: Licensed in at least 2 of FL, TX, OH, or NJ.
