Weekend Everyday Care Advanced Practice Provider (NP or PA) - Responsive Care

Location: Fully remote within one of the contiguous 50 states. Regular Saturday and Sunday work is required, with three 12-hour shifts per week from 8:00am to 8:30pm EST; a 4x10 schedule may be considered if Saturday and Sunday remain working days.

Salary: $120,000–$155,000 base annually, plus a performance-based bonus of $10,000–$20,000, for total compensation of $130,000–$170,000.

What you will do

• Conduct focused virtual assessments for patients with hypertension, hyperlipidemia, coronary artery disease, diabetes, and other chronic conditions.
• Develop individualized diagnoses and treatment plans covering medication management, lifestyle changes, and monitoring.
• Manage prescription refill queues and centralized results workflows, including review, triage, and follow-up on laboratory and diagnostic results.
• Use live digital chat and AI-assisted prescribing and triage tools, while providing feedback to improve clinical workflows and the electronic health information platform.
• Identify risks, comorbidities, contraindications, adherence issues, and medication side effects, and provide appropriate counseling.
• Coordinate care with PCPs, specialists, pharmacy, clinical nursing, social work, family members, and caregivers.

Requirements

• Advanced Practice Provider with at least three years of outpatient clinical practice experience.
• At least two years of primary care or relevant subspecialty experience focused on hypertension and lipid management, including cardiology, nephrology, neurology, endocrinology, primary care, or urgent care.
• Nurse Practitioners must hold active, good-standing RN and APRN licenses; Physician Assistants must hold an active, good-standing PA license.
• Ability to obtain licenses in requested states within 90 days of hire and additional state licenses as needed.
• Willingness to work three 12-hour shifts weekly, including both Saturday and Sunday, during EST business hours.
• Accurate medical documentation and compliance with applicable legal, ethical, privacy, and telehealth standards.

Nice to have

• Experience managing cardiometabolic conditions, including hypertension, hyperlipidemia, coronary artery disease, and diabetes.
• Virtual care, centralized results management, live digital chat, or prescription refill workflow experience.
• Experience with STARS/HEDIS measures, clinical comorbidity coding, and identifying care gaps.
• Comfort participating in testing and iterative improvement of AI-powered clinical tools.
• Proficiency in English and Spanish.
