Everyday Care Advanced Practice Provider (NP or PA) - Responsive Care

Location: Fully remote within one of the contiguous 50 states. Monday–Friday, 8-hour shifts; 8am–5pm MST/PST preferred, with EST/CST hours also considered.

Salary: $120,000–$155,000 annual base salary, plus a performance-based bonus of $10,000–$20,000; total compensation range of $130,000–$170,000.

What you will do

• Conduct focused assessments and virtual visits for patients managing hypertension, hyperlipidemia, coronary artery disease, and diabetes.
• Develop individualized treatment plans covering diagnoses, medications, lifestyle changes, monitoring, adherence, and side effects.
• Manage prescription refill queues and centralized medication renewals according to clinical protocols.
• Review, triage, and follow up on laboratory and diagnostic results using risk-tiering workflows.
• Use digital chat, AI-assisted prescribing and triage tools, and the electronic health information system while providing feedback to improve them.
• Coordinate care with primary care providers, specialists, pharmacy, nursing, social work, family members, and caregivers.

Requirements

• Active, good-standing RN and APRN licenses for Nurse Practitioners, or an active, good-standing Physician Assistant license for Physician Assistants.
• At least three years of outpatient clinical practice experience, including at least two years focused on primary care or a relevant subspecialty.
• Experience managing hypertension and lipid disorders, with knowledge of cardiovascular disease prevention and chronic care.
• Ability to obtain licenses in requested states within 90 days of hire and additional state licenses as needed; hirify.global covers licensing costs and provides administrative support.
• Must be located in one of the contiguous 50 states and able to work the required weekday schedule.

Nice to have

• Experience with cardiometabolic conditions, diabetes, coronary artery disease, medication management, and lifestyle modification.
• Virtual care, centralized results management, live digital chat, STARS/HEDIS measures, clinical coding, or care-gap identification experience.
• Experience with or interest in AI-enabled clinical workflows and iterative testing of clinical tools.
• Proficiency in English and Spanish.
