Salary: $270,000 - $300,000 USD

What you will do

• Provide remote preventive, acute, and chronic care via video visits.
• Manage a dedicated patient panel and foster longitudinal relationships.
• Execute population health initiatives, including AWVs, care gaps, and risk capture.
• Coordinate post-discharge support to improve treatment understanding and reduce readmissions.
• Collaborate with a 24/7 support team of Care Concierge and Clinical Operations to minimize administrative load.

Requirements

• Active medical licenses in Wisconsin, Minnesota, and Iowa with IMLC compact licensure.
• Board-Certification in Family Medicine.
• 3+ years of post-residency experience, including at least 2 years in an in-person primary care setting.
• Prior experience with EPIC EMR and virtual visits.
• Ability to work 3x12-hour shifts per week, specifically including Saturday and Sunday.
