Clinical Pharmacist (Medtech)

Location: Must be based in and eligible to work in the United States. This role is remote but requires on-site meetings multiple times per year.

What you will do

• Conduct Medication Therapy Management (MTM) services, including Comprehensive and Targeted Medication Reviews.
• Engage patients, providers, and pharmacies over the phone to resolve medication challenges and transitions of care.
• Evaluate and document patient data within the software platform to ensure clinical accuracy.
• Develop intervention strategies to optimize medication utilization and meet quality measures.
• Collaborate with engineering and leadership to improve platform workflows and automation.
• Coach and support team members while maintaining high productivity and service quality.

Requirements

• Doctor of Pharmacy (PharmD) degree from an accredited institution.
• Active, good-standing Pharmacist license in the United States.
• Must live in and be eligible to work in the United States.
• Knowledge of CMS quality measures, Star Ratings, HEDIS, and Medicare Part C/D.
• Strong computer skills and comfort navigating multiple browser-based systems simultaneously.
• Ability to work 40 hours/week within 6AM-6PM Pacific Time, including rotating Saturday shifts.

Nice to have

• Experience in ambulatory care, chronic disease management, or geriatric populations.
• Prior experience with MTM platforms or in a health plan/PBM setting.
• Board certification in a specialty area.
• PGY1 or PGY2 residency in a relevant clinical field.
