Telehealth Preventative Primary Care Advanced Practice Provider - NP/PA (Telehealth)

Location: Fully remote within Georgia, United States; primarily video telehealth with limited occasional in-home visits for members in the local area.

Salary: $115,000–$140,000 annually

What you will do

• Conduct primarily telehealth video visits and limited occasional in-home visits.
• Perform comprehensive assessments and provide evidence-based preventive care, chronic-condition support, medication optimization, and health maintenance.
• Identify care gaps and coordinate with primary care providers, specialists, pharmacists, nurses, caregivers, and families.
• Develop individualized, whole-person care plans and support coordinated member-centered care.
• Use integrated technology and AI-enabled workflows to reduce administrative work and improve member interaction time.
• Communicate empathetically and clearly with members and caregivers.

Requirements

• Active Nurse Practitioner/APRN or Physician Assistant license.
• Clear active NP/PA license and willingness to obtain additional licenses when necessary within the first 90 days; support and reimbursement are provided.
• At least 3 years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
• Active BLS certification at the time of hire.
• Comfort delivering care through video telehealth and using electronic documentation systems.

Nice to have

• Experience with managed care models, including STARS/HEDIS and care-gap identification.
• Experience providing comprehensive or preventive care to Medicare populations.
• Experience supporting older adults or people with complex medical or social needs.
