Location: Fully Remote - Georgia
Salary: $115,000 - $140,000 annually

What you will do
- Conduct primarily telehealth video visits, with limited and occasional in-home visits for local members.
- Perform comprehensive, evidence-based preventative and primary care assessments, including chronic condition support and health maintenance.
- Identify and address care gaps and coordinate with internal and external care team members to build individualized whole-person care plans.
- Use integrated technology and AI-enabled workflows to reduce administrative burden and maximize meaningful member interaction time.
- Communicate with empathy and clarity with members and caregivers to build trust.
Requirements
- Active Nurse Practitioner (APRN/NP) or Physician Assistant (PA) license.
- 3+ years of outpatient clinical practice, ideally in primary care, family medicine, internal medicine, or geriatrics.
- Must be willing to obtain additional licenses if necessary within the first 90 days (support and reimbursement provided).
- Active BLS certification at time of hire.
- Comfort delivering care via video telehealth and using electronic documentation systems.
Nice to have
- Experience with managed care models, including STARS/HEDIS and care gap identification.
- Experience performing comprehensive or preventive care visits with Medicare populations.
- Experience supporting older adults and/or individuals with complex medical or social needs.
