Nurse Practitioner 100% Virtual, CareBridge

Location: 100% virtual full-time role in the United States, with required in-person training and a commuting-distance requirement for the posting locations. Candidates must hold an active NP license in Ohio, New Jersey, and/or Massachusetts and an active RN Compact License. Work hours are Monday–Friday, 8:00 a.m.–5:00 p.m. EST, with rotating overnight on-call coverage 2–3 days per month.

Salary: $104,576–$189,888 per year. A $5,000 sign-on bonus is offered.

What you will do

• Provide urgent healthcare through telephone and telehealth video visits for patients receiving home and community-based services.
• Develop and implement care plans for adults with chronic, complex, and co-occurring medical and mental health conditions.
• Collect patient histories, conduct assessments, review diagnostics, and create treatment plans aligned with patient goals.
• Identify care gaps and address patients’ physical and psychosocial needs with an interdisciplinary team.
• Educate patients and families on medications, side effects, illness progression, nutrition, adherence, and crisis prevention.
• Coordinate care with physicians, providers, families, and clinical teams while maintaining compliant medical records and prescribing medications within state authority.

Requirements

• Master of Science in Nursing and active national Nurse Practitioner certification.
• Valid, current, active, unrestricted Family or Adult Nurse Practitioner license in Ohio, New Jersey, and/or Massachusetts.
• Active RN Compact License required.
• At least 2 years of experience managing complex care cases.
• Experience working with Electronic Medical Records.

Nice to have

• DEA registration or eligibility.
• Active Medicaid number in Ohio, New Jersey, and/or Massachusetts.
• Experience managing complex care for patients with developmental disabilities and chronic illnesses.
