Chronic Care Management Coach (Telehealth)

What you will do

• Perform chronic care management services for Phase 3 Pulmonary Rehab patients under nurse practitioner supervision.
• Conduct biweekly video visits and communicate with patients by phone, text, and email.
• Coach patients on health goals through accountability, motivation, and education about diseases, therapies, medications, and available resources.
• Monitor vital signs, maintain patient safety, and respond to live medical emergencies when needed.
• Coordinate appointments and medication clarifications or adjustments with physicians’ offices.
• Maintain accurate, timely, organized, and confidential medical documentation while supporting digital care delivery improvements.

Requirements

• Bachelor’s degree in Clinical Exercise Physiology or Exercise Science.
• At least two years of experience in a medical office setting, preferably in cardiac or pulmonary rehabilitation.
• Experience with medical records systems and software, medical terminology, and clinical documentation.
• Knowledge of HIPAA regulations and commitment to patient privacy and confidentiality.
• Strong organizational, problem-solving, communication, interpersonal, and data-entry skills.
• Comfort with emergencies, live video telehealth visits, fast-paced clinical changes, and technology platforms.

Nice to have

• Two or more years in a similar role.
• Remote work experience.
• ACSM Clinical Exercise Physiologist Certification or AACVPR CCRP certification.
• Registered Health Information Technician certification or a similar credential.
