What you will do

• Conduct bi-weekly video visits and communicate with patients via phone, text, and email.
• Provide coaching on patient health goals, including accountability, motivation, and encouragement.
• Monitor vital signs during sessions and handle live emergency situations as needed.
• Coordinate care for patients, including appointment scheduling and medication clarifications with provider offices.
• Provide 1:1 education on diseases, therapies, medications, and available resources.
• Collaborate with founders and the clinical team to innovate and improve the digital care delivery model.

Requirements

• Bachelor's degree in Clinical Exercise Physiology or Exercise Science.
• Must be based in the United States.
• Minimum of two years of experience in a medical office setting, preferably in cardiac and pulmonary rehab.
• Proficiency with medical records systems and software.
• Knowledge of medical terminology and HIPAA regulations.
• Strong organizational, problem-solving, and communication skills.

Nice to have

• 2+ years of experience in a similar role.
• Remote work experience.
• ACSM Clinical Exercise Physiologist Certification.
• AACVPR CCRP certification.
• Registered Health Information Technician (RHIT) or similar credential.
