📋 Description
- Travel for in-home visits to assess and modify homes
- Conduct in-home assessments and functional evaluations
- Deliver home modification recommendations
- Educate clients and caregivers on fall prevention and safe mobility
- Collaborate with clients and families on care planning
- Maintain accurate documentation in the electronic system
🎯 Requirements
- Degree in Occupational Therapy from an accredited program
- 2+ years of clinical experience
- Experience in home health, aging in place, or disability services preferred
- Strong interpersonal and clinical reasoning skills
- Proficiency with technology and digital documentation
- Ability to work independently while collaborating with a remote team
