What you will do

• Engage regularly with patients to track adherence to personalized care plans.
• Monitor patient device readings and investigate out-of-bound values.
• Maintain meticulous records of all patient interactions and program details.
• Ensure all patient enrollments and activities adhere to strict legal and ethical compliance standards.
• Track individual performance metrics to meet or exceed care goals.

Requirements

• National Certification in Medical Assisting (AAMA, NHA, NCCT, NAHP or similar) is a hard requirement.
• Must be a resident of the United States.
• Strong written and verbal communication skills.
• Basic knowledge of chronic health conditions and their management.
• Comfortable using technology for documentation and patient communication.
• Ability to work independently and as part of a remote team.
