What you will do

• Conduct telephonic outreach to assess member needs and provide appointment scheduling assistance.
• Educate members on their rights, responsibilities, and the importance of preventive health services.
• Recruit and enroll members into Disease Management Programs and refer them to Care Management.
• Coordinate access to preventive health services by contacting members, providers, and community agencies.
• Navigate members through the grievance and appeals process.
• Maintain accurate records of all contacts in computer systems for data collection.

Requirements

• High School diploma or GED.
• 1 year of experience in Community Outreach or Customer Service.
• Must be based in Pennsylvania.
• Strong active listening and communication skills for interacting with diverse populations.
• Ability to work independently and manage multiple tasks simultaneously.

Nice to have

• Associate’s Degree.
• Familiarity with healthcare or insurance.
• Experience working with Medicaid and/or Medicare populations.
