Salary: $120k–$170k (base + performance bonus).

What you will do

• Conduct comprehensive virtual primary care assessments and manage chronic conditions like hypertension and diabetes.
• Evaluate and treat acute concerns such as infections and minor injuries within a virtual care scope.
• Order and interpret diagnostic tests, imaging, and lab work while coordinating follow-up care.
• Manage medication prescriptions, titrations, and refills to ensure patient safety and adherence.
• Provide health coaching and patient education to empower individuals in managing their health.
• Collaborate with interdisciplinary teams to facilitate specialist referrals and home-based services.

Requirements

• Active RN and APRN or Physician Assistant license in good standing.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of experience in primary care, urgent care, or internal medicine.
• Ability to obtain licensure in multiple states within 90 days of hire.
• Active BLS certification.
• Must be available to work 8:30 AM – 5:00 PM PST.

Nice to have

• Experience with telehealth or virtual care workflows.
• Familiarity with value-based care models and quality metrics like HEDIS or STAR ratings.
• Proficiency with digital documentation and EHR platforms.
