Senior Transcare Nurse (Clinical Homecare)

Location: Exeter, Devon, United Kingdom, with work involving trusts and patients in Newton Abbot and surrounding areas. The role includes regular travel to support homecare patients and split shifts across Monday to Sunday, totaling 40 hours over 5 days per week.

What you will do

• Administer prescribed parenteral nutrition, IV therapies, and associated care to patients in their homes.
• Train and support patients and carers to use infusion equipment and devices independently.
• Teach patients and carers how to connect and disconnect parenteral nutrition and IV therapies.
• Provide education on the care and management of CVADs in line with local policies and national guidelines.
• Participate in the on-call rota, make autonomous decisions on out-of-hours queries, and escalate issues when required.
• Support pre-discharge assessments, home assessments, patient installations, documentation, and issue escalation.

Requirements

• Registered General Nurse with current NMC qualifications.
• At least two years of current clinical experience, with intensive care, high dependency, general surgery, or equivalent nursing experience at top Band 5 level.
• Competence in the management and delivery of parenteral nutrition and other IV therapies.
• Competence in CVAD care and management, including taking blood samples from CVADs.
• Strong communication and time-management skills, with the ability to work to strict timescales and make appropriate clinical decisions during emergencies.
• Flexibility to travel to homecare patients and carers in the Newton Abbot area is essential.

Nice to have

• Mentoring or teaching certificate, or willingness to obtain an equivalent qualification within 12 months.
