Senior Transcare Nurse (Medical): Providing individualised quality nursing care to patients requiring Parenteral Nutrition and other IV therapies in their homes with an accent on clinical support and patient education. Location: Must be based in or able to travel to Exeter and surrounding areas, United Kingdom

What you will do
- Administer prescribed IV therapies and provide associated care to patients unable to be trained to independence.
- Train and educate patients and carers on the use of infusion devices and the administration of therapies.
- Manage and care for Central Venous Access Devices (CVADs) in accordance with local policies and national guidelines.
- Participate in the on-call rota and make autonomous clinical decisions to resolve out-of-hours queries.
- Coordinate discharge management, including conducting pre-discharge and home assessment visits.
- Support the installation of new TransCare patients and ensure all relevant documentation is completed.
Requirements
- Registered General Nurse with current NMC qualifications.
- Experience in Intensive Care, High Dependency, or General Surgery at a level equal to top of Band 5.
- Minimum of 2 years of current clinical experience.
- Competency in the management and delivery of Parenteral Nutrition (PN) and other IV Therapies.
- Proficiency in blood sampling and management of CVADs.
- Flexibility to travel to support Homecare patients in Exeter and surrounding areas is essential.

- Mentoring/Teaching Certificate or equivalent (must be attained within 12 months of hiring).
