Onsite Care Coordination Nurse (Healthcare)

Location: Hybrid role based at Atrium Health Cabarrus, 920 Church Street North, Concord, North Carolina; candidates must live within a reasonable 30–60 minute commute. Onsite facility work is combined with limited remote documentation and occasional travel to hirify.global offices for training or meetings.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Conduct comprehensive face-to-face assessments of newly admitted members at assigned acute care facilities.
• Support members and families with discharge planning, follow-up appointments, diagnoses, medications, and preventive care.
• Identify care gaps, safety concerns, and discharge barriers, then develop strategies to address them.
• Coordinate care with attending providers, facility case managers, and other healthcare professionals.
• Complete accurate and timely documentation while following infection control, safety, and patient privacy standards.

Requirements

• Current, unrestricted Registered Nurse license in North Carolina.
• Residence within a 30–60 minute driving distance of Atrium Health Cabarrus in Concord, North Carolina.
• At least three years of recent acute care nursing experience.
• Experience with discharge planning or care coordination in a hospital or health plan setting.
• Experience with Microsoft Word, Outlook, Excel, and electronic medical records.
• Valid driver’s license, dependable automobile, personal vehicle liability insurance, TB screening, and required background and facility credentialing clearances.

Nice to have

• Bachelor’s degree in Nursing.
• Experience in care management, health promotion, coaching, utilization management, or health plans.
• Care management or related specialty certification.
• Knowledge of HEDIS and Stars measures.
• Bilingual English/Spanish skills.
