Nurse Care Manager, Adult

Location: Remote within the United States; must reside in a compact NLC state. Working hours are 9:00 AM–6:00 PM PST.

Salary: $73,700–$103,180 annual base salary, plus equity and benefits.

What you will do

• Deliver coordinated, patient-centered care management by telephone and video.
• Create care plans with members and a multidisciplinary clinical team.
• Help members navigate complex medical conditions, treatment pathways, benefits, and healthcare resources.
• Coordinate care with local providers and provide compassionate longitudinal follow-up.
• Support members and families during hospitalizations, rehabilitation stays, and other acute healthcare episodes.
• Coordinate clinical and social resources to address members’ needs holistically.

Requirements

• Bachelor of Science in Nursing degree.
• At least 5 years of nursing experience and 2 years of care management experience.
• Residence in a compact NLC state and an active Compact RN license in good standing are required.
• Willingness to obtain and maintain licensure in multiple states.
• Experience supporting multiple medical conditions and patient populations across age ranges.
• Experience working remotely, using multiple computer and medical record systems, communicating with patients and families, documenting clinical information clearly, and following HIPAA and security regulations.
