What you will do

• Deliver coordinated virtual care management by phone or video to improve health outcomes.
• Create care plans with members and diverse care team to achieve goals.
• Guide members through complex conditions, treatments, benefits, and healthcare system.
• Partner with local providers for coordinated care.
• Provide compassionate longitudinal follow-up and build supportive relationships.
• Assist during acute episodes like hospitalizations with case management.
• Coordinate clinical and social resources holistically.

Requirements

• Bachelor of Science in Nursing (BSN).
• 5+ years nursing experience; 2+ years in care management.
• Active Compact RN license in good standing; reside in compact NLC state.
• Willingness to maintain licensure in multiple states.
• Work until 9-6PM PST; comfortable with various medical conditions across age ranges.
• Experience working remotely; proficiency with computer/medical record systems.
• Empathetic, efficient in clinical history gathering, patient-level communication, and HIPAA compliance.
