Registered Nurse Care Manager

Location: Remote from Wisconsin, supporting members in Lincoln, Langlade, Oneida, Forest, and Vilas counties. Travel to member locations is required for approximately 40% of the role, with occasional travel to hirify.global offices for training or meetings.

Salary: $71,100–$97,800 per year, plus eligibility for a bonus incentive plan.

What you will do

• Assess members' health and safety needs and develop comprehensive Member Care Plans.
• Conduct regular health assessments and monthly phone contacts.
• Coordinate care with acute and primary care providers.
• Educate members about disease processes, risks, nutrition, exercise, and lifestyle modifications.
• Travel to members' settings for in-person visits and document care activities within required DHS contract timeframes.

Requirements

• Associate degree in nursing.
• Active Registered Nurse license in Wisconsin in good standing.
• Residence within 45 minutes of the assigned coverage area is preferred.
• Valid state driver's license and personal vehicle liability insurance with minimum limits of 100/300/100.
• TB screening is required because the role is patient-facing.
• Ability to work Monday through Friday, 8:00 a.m.–4:30 p.m. CST, with a 40-hour workweek.

Nice to have

• At least one year of care management experience.
• Previous care management experience.
