What you will do

• Conduct virtual adult and pediatric wellness visits and health assessments.
• Perform health risk assessments, medication reviews, and cognitive screenings.
• Collaborate with in-home clinical team members to evaluate vital signs and arrange blood draws.
• Provide comprehensive care coordination, including preventive interventions and specialist referrals.
• Document findings accurately using mobile technology and secure messaging apps.

Requirements

• Board Certified as a Family Nurse Practitioner
• Active Family Nurse Practitioner License in Oklahoma
• Ability to work independently and in a team environment
• Strong written and verbal communication skills
• Technologically savvy with experience using mobile devices for charting
• Must pass a national background check and clinical license verification
