Location: Must be based in the USA and willing to work 8-5 in CST, MST, or PST. Active RN and APRN or PA license in AL, AZ, GA, IL, OH, or TX required at time of hire.

Salary: $120K–$155K base range (Total compensation $130K–$170K including performance bonus).

What you will do

• Conduct thorough virtual cardiology assessments and develop individualized treatment plans.
• Manage guideline-directed medical therapies for heart failure patients.
• Collaborate with an interdisciplinary team of physicians, pharmacists, and social workers.
• Order and interpret diagnostics including echocardiograms, EKGs, and Holters.
• Provide proactive management of comorbid conditions to prevent hospitalizations.
• Utilize and provide feedback on proprietary electronic health information systems.

Requirements

• NP or PA with 5+ years of cardiology clinical practice experience.
• Bilingual in English and Spanish (must be comfortable conducting clinical assessments in both).
• Active and clear RN and APRN or PA license in AL, AZ, GA, IL, OH, or TX.
• Willingness to work 8-5 in CST, MST, or PST time zones.
• Strong experience with telehealth platforms and comfort with modern healthcare technology.
• Secure, private workspace with reliable internet connection.

Nice to have

• Experience in an outpatient cardiology setting (ideally 2+ years).
• Cardiovascular Nurse Practitioner Certification.
• Understanding of managed care, STARS/HEDIS measures, and clinical coding.
