Advanced Practice Provider (Cardiology): Delivering virtual-first primary cardiology care to seniors with an accent on heart failure management and guideline-directed medical therapy. Location: Must be based in the USA and willing to work 8-5 in CST, MST, or PST. Active RN and APRN or PA license in AL, AZ, GA, IL, OH, or TX required at time of hire.
Salary: $120K–$155K base range (Total compensation $130K–$170K including performance bonus).

What you will do
- Conduct thorough virtual cardiology assessments and develop individualized treatment plans.
- Manage guideline-directed medical therapies for heart failure patients.
- Collaborate with an interdisciplinary team of physicians, pharmacists, and social workers.
- Order and interpret diagnostics including echocardiograms, EKGs, and Holters.
- Provide proactive management of comorbid conditions to prevent hospitalizations.
- Utilize and provide feedback on proprietary electronic health information systems.
Requirements
- NP or PA with 5+ years of cardiology clinical practice experience.
- Bilingual in English and Spanish (must be comfortable conducting clinical assessments in both).
- Active and clear RN and APRN or PA license in AL, AZ, GA, IL, OH, or TX.
- Willingness to work 8-5 in CST, MST, or PST time zones.
- Strong experience with telehealth platforms and comfort with modern healthcare technology.
- Secure, private workspace with reliable internet connection.
Nice to have
- Experience in an outpatient cardiology setting (ideally 2+ years).
- Cardiovascular Nurse Practitioner Certification.
- Understanding of managed care, STARS/HEDIS measures, and clinical coding.
