Location: Remote (USA). Must be able to work Monday-Friday, 8:30 AM - 5:00 PM MST/PST.
Salary: $120,000–$155,000 base range (Total compensation $130,000–$170,000 including bonus).

What you will do
- Perform comprehensive post-discharge medical assessments and medication reconciliations.
- Develop and implement individualized care plans for patients transitioning from acute settings.
- Provide virtual urgent care visits to address acute symptoms and manage disease exacerbations.
- Collaborate with an interdisciplinary team of RNs, case managers, and medical directors.
- Participate in interdisciplinary rounds to identify and support high-risk patients.
- Contribute to quality improvement projects and provide feedback on clinical information systems.
Requirements
- Must be bilingual in Spanish and English.
- Must be available to work 8:30 AM - 5:00 PM MST/PST.
- Active and in good standing RN/APRN or Physician Assistant license.
- At least 3 years of experience in hospital, post-hospital, or primary care settings.
- Active BLS certification.
- Willingness to obtain state licenses as required by the company within 90 days.
Nice to have
- Experience in internal medicine, urgent care, emergency room, or geriatrics.
- Experience performing visits over telehealth video platforms.
- Expertise in managing CHF, diabetic emergencies, COPD, and hypertensive emergencies.
