TL;DR

Community Health Navigator: Assisting pediatric patients with complex medical needs in post-hospital transitions through telephonic and in-person support with an accent on social determinants of health and care coordination. Focus on ensuring continuity of care, liaising with hospital discharge planners, and delivering essential resources to families.

Location: Remote (US), with a strong preference for candidates based in TX, FL, MO, NC, GA, TN, or NY. Must be available to work Monday-Friday, 9:00am-6:00pm ET or 8:00am-5:00pm CT.

Salary: $23–$28 per hour

What you will do

  • Collaborate with an interdisciplinary team to address patient needs post-discharge.
  • Conduct outreach to patients discharged from emergency rooms to ensure continuity of care.
  • Coordinate with facility discharge planners to remove barriers to after-hospital resources.
  • Provide in-person community support, including pharmacy coordination and home visits.
  • Liaise with hospitals and health systems to promote program awareness.
  • Attend daily team huddles to communicate patient progress and needs.

Requirements

  • Bachelor’s degree required.
  • 3+ years of experience in a healthcare setting (patient-facing hospital or facility role).
  • Strong preference for bilingual (Spanish) proficiency.
  • Ability to work within the specified US time zones (ET/CT).
  • Passion for pediatric healthcare and supporting children with medical complexity.

Nice to have

  • National CHW certification.
  • CPR certification.
  • Experience in an ER setting.
