What you will do

• Engage patients via video messaging, consultations, and text to enroll them in the adherence program.
• Deliver personalized education and coaching through daily asynchronous video check-ins.
• Perform medication reconciliation and monitor administration techniques via video submissions.
• Create short-form educational and motivational videos for patient engagement and population health.
• Assess medical, behavioral, and social barriers to health and escalate complex cases.
• Collaborate with provider offices and pharmacies to remove barriers to patient care.

Requirements

• Active Registered Nurse (RN) license in good standing.
• Minimum 5 years of nursing experience.
• 5 years of experience in patient engagement, health education, or care coordination.
• Bilingual in Spanish.
• Must be eligible to work in the United States (no visa sponsorship available).
• Availability to work evenings and weekends.

Nice to have

• Experience in telehealth, community health, or digital health settings.
• Certification in health coaching.
• Knowledge of Directly Observed Therapy (DOT) models.
• Experience working with Medicaid and Medicare Dual Eligible (D-SNP) populations.
