Pediatric Advanced Practice Provider

TL;DR

Pediatric Advanced Practice Provider (NP/PA): Deliver virtual-first pediatric care for children with special health care needs, managing complex patient panels and coordinating multidisciplinary treatment plans with an accent on continuity of care, virtual enrollment and acute escalations, and reducing unnecessary visits and hospitalizations. Focus on clinical decision-making for complex pediatric cases, care coordination with PCPs and specialists, and bridging care gaps through prescriptions and streamlined referrals.

Location: Remote (United States) — schedule 4x10s, 8:00 am–6:00 pm (Tuesday–Friday). Active LA, TN, MO, or TX licensure required.

Salary: $115,000+ (plus annual bonus and benefits)

What you will do

• Manage a panel of complex pediatric patients, overseeing care plans and coordinating with PCPs and specialists.
• Provide wrap-around, multidisciplinary virtual care with pediatricians, registered nurses, pharmacists, dietitians, and social workers.
• Educate and support families to help them navigate their child’s health journey and improve long-term outcomes.
• Deliver high-quality virtual care including enrollment visits, sick visits, acute escalations, and post-hospital follow-ups, plus in-home virtual assessments via the Mobile Integrated Care team.
• Ensure continuity of care by bridging care gaps, coordinating prescriptions, and streamlining referrals.

Requirements

• Active TX, LA, MO, or TN licensure required.
• Minimum 5 years of experience with a pediatric population (community-based health or complex care strongly preferred).
• Completed an accredited NP or PA program and hold an unrestricted NP or PA state license, including board certification.
• Experience and comfort working with children with complex medical needs.
• Ability to lead and guide a clinical team.
• Bilingual Spanish is strongly preferred.
