What you will do

• Act as a liaison between healthcare providers and members to coordinate care and improve plan adherence.
• Manage order coordination for DME, home health services, lab testing, and imaging.
• Prepare members for telehealth visits and provide ongoing patient education.
• Maintain accurate medical records and track test results for clinician review.
• Advocate for members by answering inquiries and coordinating follow-up care.

Requirements

• Active or expired MA Certification (or equivalent experience with diploma/transcripts).
• Fully bilingual in English and Spanish.
• Minimum 2 years of patient-facing experience.
• Ability to work within US time zones (ET schedule).
• Strong organizational skills and comfort with electronic health records and digital platforms.

Nice to have

• Prior experience in telehealth or primary care settings.
• Experience in care coordination for Medicare populations.
• Knowledge of closing preventative health screenings and care gaps.
