What you will do

• Coordinate durable medical equipment, home health services, lab testing, and imaging for members.
• Support clinicians by gathering medical records, confirming appointments, and managing outbound communications.
• Educate members on care plans and assist with telehealth visit onboarding.
• Serve as a primary point of contact and advocate for members regarding care coordination.
• Track and follow up on lab results to ensure timely review by clinical providers.
• Maintain accurate member records and manage large member panels in a fast-paced environment.

Requirements

• Active or expired MA Certification (or equivalent diploma/transcripts).
• Fully bilingual in English and Spanish (C1+ proficiency).
• Two years of patient-facing experience.
• Must be based in the United States.
• Strong organizational skills and ability to manage 100+ member panels.
• Proficiency with electronic health records and computer platforms like Google Workspace.
