What you will do

• Screen members for Hypertension Clinic eligibility and provide guidance on blood pressure monitoring.
• Conduct comprehensive assessments of medical history, medication adherence, and lifestyle habits.
• Deliver intensive counseling on nutrition, weight management, physical activity, and smoking cessation.
• Utilize motivational interviewing to influence positive, sustainable behavioral changes in members.
• Coordinate care across multidisciplinary teams including NPs, pharmacists, and social workers.
• Maintain accurate documentation and communicate care plans with primary care providers.

Requirements

• Certified health coach required
• At least 2 years of experience in longitudinal chronic disease management
• Must be based in the United States
• Experience caring for Medicare patients
• Strong communication and organizational skills with the ability to multitask in a virtual environment
• Spanish bilingual skills are highly desired
