What you will do

• Screen members for eligibility in the Hypertension Clinic and serve as their primary point of contact throughout enrollment.
• Provide blood pressure monitors, assess measurement technique, and reinforce accurate self-monitoring.
• Conduct hypertension assessments covering medical history, medications, adherence, and lifestyle habits.
• Deliver lifestyle and nutrition counseling focused on sodium and alcohol reduction, weight management, physical activity, and smoking cessation.
• Coach members using motivational interviewing to set realistic goals and sustain healthy behavior changes.
• Coordinate care with multidisciplinary Devoted teams and external providers, document interactions, schedule follow-ups, conduct outreach, and close applicable Stars measure gaps.

Requirements

• Certified health coach certification required.
• At least 2 years of experience in longitudinal chronic disease management.
• Basic understanding of hypertension and/or cardiovascular disease.
• Ability to counsel members on therapeutic lifestyle modifications and motivate behavioral change.
• Strong communication, organization, prioritization, multitasking, and problem-solving skills.
• Experience working as part of a multidisciplinary care team.

Nice to have

• Spanish bilingual skills.
• Experience caring for Medicare patients.
