What you will do

• Facilitate patient enrollment into Remote Therapeutic Monitoring (RTM) and Chronic Care Management (CCM) programs.
• Conduct daily calls and text messages to document patient health status and maintain engagement.
• Collect and document patient health information, including histories and new symptoms.
• Support patients by providing health pointers, such as stretches and meditation videos.
• Maintain strict confidentiality and adherence to HIPAA regulations and company policies.

Requirements

• Certification as an LPN.
• 3+ years of experience in office or remote healthcare settings.
• Proficiency in Spanish is required.
• Strict commitment to HIPAA regulations and patient privacy.
• Proficiency with Zoom, Slack, and online enrollment platforms.
• Access to a laptop with a working microphone, camera, and reliable internet in a quiet workspace.

Nice to have

• Experience in Neurology, Orthopedics, Physical & Rehabilitation Medicine, or Sports Medicine.
