What you will do

• Conduct screenings, assessments, and home/center visits for infants and toddlers.
• Utilize a parent coaching model to empower families in their child's development.
• Collaborate within a trans-disciplinary team and with community providers to ensure continuity of care.
• Coordinate the development, monitoring, and review of Individual Family Service Plans.
• Provide home-based services to families across the designated catchment area.

Requirements

• Current licensure as a Physical Therapist by the Massachusetts Board of Registration in Allied Professional Health.
• Valid driver’s license and reliable transportation is required.
• Preferably two years of experience in pediatrics.
• Proficiency in Word, Excel, Adobe, and Office365 (Outlook, OneDrive, Teams).
• Effective verbal and written communication skills.
