What you will do

• Utilize a parent coaching model to support families in child development.
• Participate in screenings, assessments, and group sessions as part of a trans-disciplinary team.
• Provide home-based services to families within the designated catchment area.
• Collaborate with community providers, including medical and social service agencies, to ensure service continuity.
• Coordinate the development and monitoring of Individual Family Service Plans.

Requirements

• Current licensure as a Physical Therapist by the Massachusetts Board of Registration in Allied Professional Health.
• Valid driver’s license and reliable transportation required for home visits.
• Two years of pediatric experience preferred.
• Proficiency with Microsoft Office 365, including Outlook, OneDrive, and Teams.
• Effective verbal and written communication skills.
