What you will do
- Provide real-time clinical support and triage for newly re-engaged pediatric patients to ensure immediate access to care.
- Perform comprehensive assessments of clinical, psychosocial, and financial needs to establish milestones and goals.
- Collaborate with physicians, APPs, and interdisciplinary teams to develop and execute individualized care plans.
- Coordinate with behavioral health and resource specialists to address complex patient needs and ensure appropriate referrals.
- Document all patient interactions, assessments, and interventions accurately and in a timely manner.
- Meet monthly productivity and engagement goals to ensure longitudinal care outcomes.
Requirements
- Licensed RN in at least one US state (eligibility to register for other state licensures required).
- Bilingual Spanish proficiency is required.
- Minimum 1 year of experience in care coordination or case management.
- Familiarity with Medicaid regulations and services.
- Must be based in the US and available for a schedule of 4x10s in EST or CST, including 1-2 Saturdays per month.
- Bachelor’s degree in nursing (BSN) from an accredited university preferred.
