What you will do

• Perform comprehensive psychosocial assessments of patients and families to recommend intervention plans.
• Provide interventions for reactions to illness, disability, psychiatric symptoms, and chemical dependency.
• Manage discharge planning through placement coordination and identification of post-hospital care needs.
• Deliver crisis intervention and protective services for elderly, homeless, or victims of abuse and neglect.
• Collaborate with multidisciplinary teams to ensure consensus on patient care and discharge goals.
• Analyze financial indicators such as length of stay (LOS) and readmission rates to drive performance improvement.

Requirements

• Licensure in Illinois: Licensed Clinical Social Worker (LCSW)
• Master's Degree in Social Work from a CSWE accredited school.
• Minimum of two years post-graduate experience in hospital social work or related settings.
• High level of interpersonal skills to affect positive patient outcomes.
• Strong organizational skills to prioritize and manage a patient caseload.
