What you will do

• Conduct comprehensive psychosocial assessments to identify emotional and social needs of patients and families.
• Connect patients and families with community resources to address complex social needs.
• Collaborate with oncologists and nurses to ensure coordinated care and treatment adherence.
• Advocate for patients' needs with insurance companies and healthcare providers to access necessary treatment.
• Provide immediate crisis intervention for sudden changes in prognosis or end-of-life decisions.
• Lead support groups and provide education on cancer-related topics for patients and their families.

Requirements

• Master's degree in Social Work from a CSWE accredited program.
• Valid and current master’s level license (LMSW, LISW, LCSW) in current practice state.
• 3-5 years of experience in a healthcare setting (oncology, palliative, or hospice preferred).
• Willingness to obtain additional state licenses as needed.
• Proficiency with technology including Google Suite, Slack, and electronic health records.
• Availability to work 8:30 AM - 5:00 PM EST.

Nice to have

• OSW-C or APHSW-C certifications.
