Pharmacy Technician (Population Health)

What you will do

• Conduct chart reviews and patient interviews to identify and resolve medication adherence issues.
• Perform medication history reconciliation for discharged patients and update electronic health records.
• Provide patient education on medication administration, adherence, and safe disposal.
• Generate data reports to track clinical pharmacy quality metrics and pharmaceutical costs.
• Proactively contact patients identified through high-risk or medication adherence reports.
• Collaborate with medical staff and escalate complex clinical issues to pharmacists.

Requirements

• Must hold a valid Michigan Board of Pharmacy Technician registration/license.
• Must be a Certified Pharmacy Technician (CPhT) with PTCB.
• Minimum of three years of pharmacy technician practice.
• High School Diploma, GED, or equivalent.
• Strong verbal and written communication skills in English.
• Ability to work independently and collaboratively within a network environment.

Nice to have

• Graduate of a formal pharmacy technician training program.
• BLS certification.
• Experience with patient outreach programs or sales.
• Knowledge of managed care, retail, or specialty pharmacy practice.
