Focus on managing complex chronic conditions remotely, coordinating labs, imaging, referrals, and home-based services, and improving digital care workflows.
Location: Fully remote within Washington, United States; Pacific Time (PST) hours required

What you will do
- Provide virtual primary care visits, including comprehensive assessments, preventive screenings, acute care, and chronic disease management.
- Diagnose and manage conditions such as hypertension, diabetes, COPD, and heart disease, creating individualized care plans.
- Evaluate acute concerns, prescribe and manage medications, and support adherence and patient safety.
- Order and interpret laboratory tests, imaging, and other diagnostics, coordinating follow-up care and specialist referrals.
- Provide health coaching, lifestyle guidance, patient education, and coordination of home-based or community services.
- Participate in clinical reviews and interdisciplinary collaboration while providing feedback to improve the electronic health platform and virtual care workflows.
Requirements
- Fully bilingual Spanish and English communication skills required.
- Active Nurse Practitioner/APRN license in good standing, with the ability to obtain licenses in multiple states within 90 days of hire.
- Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
- At least two years of primary care, urgent care, or internal medicine experience.
- Active BLS certification at the time of hire.
- Availability for Pacific Time hours, flexible shifts aligned with patients’ local business hours, and potential weekend shifts.
Nice to have
- Telehealth or virtual primary care experience.
- Experience with value-based care, population health management, HEDIS, or STAR ratings.
- Familiarity with EHRs, digital documentation, and telemedicine workflows.
