What you will do

• Provide virtual primary care visits, including comprehensive assessments, chronic disease management, and acute care interventions.
• Diagnose and manage chronic conditions such as hypertension, diabetes, COPD, and heart disease.
• Order and interpret lab tests and imaging, coordinating necessary follow-up care.
• Manage medication prescriptions, titrations, and refills while ensuring patient safety.
• Provide health coaching and lifestyle guidance to empower patients in managing their health.
• Utilize and provide feedback on the electronic health platform to improve provider and patient experience.

Requirements

• Fully bilingual in Spanish and English
• Ability to work Pacific Time (PST) hours
• Active Nurse Practitioner (APRN) license and ability to obtain licensure in multiple states within 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of experience in primary care, urgent care, or internal medicine.
• Active BLS certification at time of hire.

Nice to have

• Experience in telehealth or virtual care settings.
• Familiarity with value-based care models and quality metrics (HEDIS, STAR ratings).
• Proficiency with EHRs and telemedicine platforms.
