Bilingual Spanish/English Routine Primary Care APP (NP/PA) - Telehealth (Telehealth)

Location: Fully remote within Arizona, United States; working Pacific Time hours is required, generally 9:00 AM–5:00 PM, with possible weekend shifts.

Salary: $120,000–$155,000 annual base salary, plus a performance-based bonus.

What you will do

• Provide virtual primary care visits, including comprehensive assessments, preventive screenings, chronic disease management, and acute care interventions.
• Diagnose and manage conditions such as hypertension, diabetes, COPD, and heart disease while developing individualized care plans.
• Evaluate acute concerns, order and interpret diagnostics, and coordinate follow-up care, specialist referrals, laboratory work, and imaging.
• Manage prescriptions, medication titrations, refills, adherence, and patient safety.
• Provide health coaching, lifestyle guidance, and patient education.
• Participate in case reviews, interdisciplinary collaboration, team huddles, and improvements to the electronic health platform.

Requirements

• Fluency in Spanish and English is required.
• Active Nurse Practitioner/APRN or Physician Assistant/PA license in good standing, with the ability to obtain licenses in multiple states within 90 days of hire.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• At least two years of primary care, urgent care, or internal medicine experience; telehealth experience is preferred.
• Active BLS certification at the time of hire.
• Flexibility to work Pacific Time business hours and potentially weekends.

Nice to have

• Experience providing chronic disease management, preventive care, and acute care through telehealth.
• Familiarity with value-based care, population health management, HEDIS, or STAR ratings.
• Experience with EHRs, digital documentation, telemedicine platforms, and virtual care workflows.
