What you will do

• Conduct virtual primary care visits, including comprehensive assessments and acute care interventions.
• Manage chronic conditions like hypertension and diabetes with individualized care plans.
• Order and interpret diagnostic tests and coordinate follow-up care with specialists.
• Handle medication prescriptions and ensure patient safety and adherence.
• Provide health coaching and lifestyle guidance to empower patients.
• Collaborate with a multidisciplinary team and provide feedback on the electronic health platform.

Requirements

• Active Nurse Practitioner (APRN) or Physician Assistant (PA) license.
• Full bilingual proficiency in Spanish and English.
• Board certification in Family Medicine, Adult-Gerontology, or Internal Medicine.
• Minimum 2 years of experience in primary, urgent, or internal medicine.
• Active BLS certification.
• Ability to obtain licensure in multiple states within 90 days of hire.
