What you will do

• Manage a defined panel of patients, serving as the primary point of contact for patients, families, and caregivers.
• Coordinate with scheduling and nursing teams to ensure service delivery and appropriate caregiver assignments.
• Proactively identify risks such as service interruptions or caregiver instability and propose effective solutions.
• Maintain accurate and timely HIPAA-compliant documentation of all interactions and submit required daily/weekly reports.
• Support the onboarding and transition of new patients and provide necessary education to aides.
• Collaborate with internal teams to ensure all services meet regulatory and agency compliance standards.

Requirements

• Fluent bilingual proficiency in Spanish and English.
• High School Diploma required; college or healthcare-related education preferred.
• Minimum 1-2 years of experience in a healthcare or administrative setting.
• Ability to work Eastern Time (9am to 6pm Mon-Fri).
• Basic computer proficiency and strong organizational and interpersonal skills.
• Ability to apply sound judgment in routine and moderately complex situations.
