What you will do

• Partner with Providers before, during, and after visits to support primary patient care.
• Perform phlebotomy, take vitals, and carry out essential medical tasks as directed.
• Coordinate patient data and documentation through electronic health records (EHR) systems.
• Support outreach efforts to schedule visits and strengthen patient engagement.
• Maintain medical supplies to ensure smooth and well-prepared home visits.
• Drive independently to conduct member visits in their homes.

Requirements

• Medical Assistant certification from an accredited program.
• At least 1 year of healthcare experience.
• Basic Life Support (BLS) certification or the ability to obtain it within 30 days of hire.
• Fluent in both English and Spanish (required).
• Must live in or near Hudson County, New Jersey.
• Valid driver’s license and reliable transportation.
