Outreach Representative (Healthcare)

What you will do

• Act as a primary liaison between members, the Health Plan, and state or federal regulators.
• Conduct telephonic outreach to assess member needs and provide scheduling assistance.
• Educate members on their rights, responsibilities, and the importance of targeted preventive health services.
• Guide members through the grievance and appeals process to represent their interests.
• Assess eligibility and enroll members into Disease Management Programs and Care Management interventions.
• Coordinate with providers and community agencies to ensure access to necessary health services.

Requirements

• High School diploma or GED.
• 1 year of experience in Community Outreach or Customer Service.
• Strong active listening skills and the ability to act sensitively toward diverse populations.
• Ability to manage multiple tasks simultaneously with sound judgment and discretion.
• Must be located in Delaware.

Nice to have

• Associate’s Degree.
• Experience working with Medicaid and/or Medicare populations.
• Familiarity with the healthcare and insurance industry.
